#8903: Continuous Quality Improvement - Propogate Notes checkbox remains ticked

The default setting of the 'Propogate Notes Tickbox' is set to be ticked upon creation of a new continuous improvement record. 

If a user unticks this box, this change will now be retained within the record upon saving the changes made.
